New patients

I AM CURRENTLY ACCEPTING NEW PATIENTS. At this time, I am only accepting adult patients (18+) who would like to meet exclusively via telehealth. Please indicate you understand these conditions when registering below.

  • Please check which insurance companies with which I am currently in-network (here) prior to registering to ensure I am contracted with your insurance company. Otherwise, you will need to use your out-of-network benefits or pay privately.

  • If you are interested in becoming a new patient, please register for my patient portal, below.

    • After selecting "Book an Appointment," scroll to the bottom right and select "Continue Without Scheduling An Appointment" in order to register (you will not be able to see any available appointments until later in the process - see below).

    • You will be asked to provide some contact and insurance information, and a brief statement about the services you are seeking ("Reason for Registering").

  • If I am able to see you, you will be emailed later with additional instructions to complete some online forms.

  • Once everything has been reviewed by staff, you will be messaged via the portal to confirm your understanding of your financial responsibilities. At that time, you will then be granted access to self-schedule appointments online.

  • If you are registering a child or adolescent as a new patient, please provide their information, NOT your own, below.
