Become a New Patient

New patients

I am currently accepting new patients. Please note I have open self-scheduling for patients, with very limited availability outside school hours. I am accepting prospective patients based on select presenting concerns to balance out my clinical load. Please be sure to include the reason you are registering (see below), as that will be the basis for considering you as a patient.

Please begin by following the directions and registering below:

PLEASE READ THESE INSTRUCTIONS FULLY

If you are interested in getting established as a patient, please register for my patient portal after reading these instructions. You will not be able to see any available appointments in the self-scheduling system in the portal until all these steps have been completed.

After going to the patient portal, select "Register."
You will be asked to provide some contact information and a brief statement about why you are seeking services ("Reason for Registering").
Next, you will be asked to verify your identity via phone or email.
Next, please enter your insurance information.
Next, you will be asked for payment information.
Next, select "create a username and password." Remember your login credentials, as the patient portal will give you access to all future scheduling, communication, and billing needs.
If you receive an error message during the registration process, we will still receive your registration information and will send you a link for completing the process later.
If I am able to see you, you will be emailed later with additional instructions to complete some online forms.
Once you have completed those online forms, everything will be reviewed by staff.
You will then be emailed to confirm your understanding of your financial responsibilities.
At that time, you will be granted access to self-schedule appointments online. You will not be able to see any available appointments in the self-scheduling system in the portal until all these steps have been completed.
